ORIGINAL STATEMENT
PLEASE DO NOT ISSUE OUR STATEMENTS WITH WRONG INFORMATION
As for Nishi-F Co., Ltd. authorized inspectors, all inspectors must be able to difine each types of our lifeboats and hook models. Accurate information must be indicated on our original statements. Unfortunatelly, we are still receiving copies of own format statements and checklists with thousands of wrong information from our authorized inspectors at overseas.
WE DO NOT SUPPORT INSPECTIONS DONE BY THIRD PARTIES
Nishi-F Co., Ltd. support Nishi-F authorized companies. We do not sell any spare parts nor support any inspections which are prosecuted by third parties. Please understand our policy to preserve authorized companies' management at each area.
CONTRACT THE FIRST
For JSMQA engineer training course completors, please be agreed with maker service agreement the first. All Nishi-F supported companies have signed service agreements at the beginning.

| ORIGINAL STATEMENT |
| For all Nishi-F Co., Ltd. authorized inspectors! Please use our original statements as we agreed on our service contracts. Own format statement shall be invalid as for the official document. |
| Please indicate the appropriate lifeboat type. Also write if the lifeboat is totall enclosed, partially enclosed, opened. Correct information will help inspectors at the next year. |

| Check the name plate inside lifeboat and write down the same manufactured date. |
| Manufacturer is pre-printed as NISHI-F CO., LTD. even it's orginally ex-Ishihara Dockyard Co., Ltd. and ex-Nishi-Nippon F.R.P. Shipbuilding Co., Ltd. |

| For lifting hook type, please indicate if it's ON-Load Release, OFF-Load, ON/OFF-Load and FIXED mechanism. These category must be informed to vessel's crew on board for a lifeboat safe use. |
| Place and date of the inspection and an annual inspection or a 5 year's inspection should be mentioned when if inspectors confirmed all the proper function on Nishi-F brand name lifeboats. |
| Issuing the statement without inspectors responsibility is meaningless. Please execute your duty for a genuine safety. Being lifeboat at a risk, we imagine you won't sleep well nights. |

| Copy of the authorized valid license I.D. card must be put on the statement. Please do not supervise somebody who are not trained with us. |
| Please also indicate your belonging company which will take all your responsibility with the certain insurance. |
| Inspector's name must be clearly indicated to anyone else who are not you. |
| Write down lifeboats serial numbers which were typed on name plates at each lifeboats. We sometimes see a vessel equipped with different lifeboat makers due to some reasons. |
| Our original statements are registered with statement serial numbers. Original statements must be prepared in advance by consenting Nishi-F Co., Ltd. In case you have to issue the temporary statement, such temporary statement must be replaced with the original maker statement. |
